Mother of Mercy 2013-2014 Admissions Application
In order to be considered for admission, the class of 2017 must complete and return this application to Mercy. The Release of Records form is to be submitted to your current school in order for your academic records to arrive at Mercy by Friday, December 7. Schedule a Shadow Visit
Shadow visits can now be scheduled online or by calling Cara Hyland, Director of Admissions (513.661.2740 ext.346) to make an appointment. You may also print out the parental consent form and photo release needed for your shadowing day at Mercy by clicking here. For more information on our shadowing program, click here.


Photo Release/Parental Consent Form
This form is needed for any grade school student attending an event at Mercy, such as the recruiting events or shadowing days on campus.

Tuition Assistance/Financial Aid
In order to be considered for tuition assistance, the PSAS form and financial documents requested in this form are due to Priviate School Aid Service by November 17, 2012. You can also fill out the form electronically on PSAS.org. Mercy's school code is 3813.

High School Placement Test Registration and Accommodation Authorization
Complete this form if you are interested in having your daughter take the HSPT with accommodations. The form gives Mercy permission to release your daughter’s accommodation information to the other schools that are receiving her High School Placement Tests scores.
